While operating of Okinawa, USS O'Brien (Cdr. William Woddward Outerbridge) in heavily damaged by a Japanese kamikaze aircraft in position 26°16'N, 127°26'E. 53 Of the crew were killed and 73 wounded and amongst them the Commanding officer Outerbridge.

25 Apr 1945
USS O'Brien (Cdr. W.W. Outerbridge) arrived at the Mare Island Navy Yard for repairs.
